The service aims to give advice and practical support to older people who are having difficulties being discharged from Hospital, due to their housing conditions.

Health through warmth

The scheme aims to improve levels of warmth, comfort and quality of life for vulnerable people who have illnesses made worse by cold and /or damp living conditions.

Crisis Support

This service assists vulnerable adults and clients who have low -moderate learning difficulties that are finding it hard to maintain their own home, manage finances or do not have the support of family or friends.
